Informed Consent for Telehealth Services

By using the Phlex Meds platform and submitting your intake form, you acknowledge and consent to the following regarding your telehealth care:

1. Nature of Telehealth

Telehealth involves the delivery of health care services using electronic communications, information technology, or other means between a healthcare provider and a patient who are not in the same physical location. Telehealth may be used for diagnosis, treatment, follow-up, and patient education. During your telehealth visit, details of your medical history and personal health information may be discussed with your provider via electronic systems.

2. Benefits of Telehealth

Telehealth may improve access to medical care by enabling you to remain at home while the physician evaluates, diagnoses, and treats you. It eliminates the need for in-person office visits for many conditions, reduces travel time, and provides timely access to medical professionals.

3. Risks of Telehealth

As with any medical procedure, there are potential risks, including but not limited to: (a) limitations of the technology may result in incomplete, delayed, or failed transmission of medical information; (b) a lack of physical examination may result in misdiagnosis; (c) delays in evaluation or treatment may occur due to technology failures; (d) security protocols could fail, resulting in a breach of personal health information. In rare cases, a provider may determine that telehealth is not appropriate for your condition and may refer you to an in-person provider.

4. Consent to Treatment

You understand that your physician will evaluate whether your symptoms and medical history are appropriate for the treatments offered through Phlex Meds. You acknowledge that no treatment will be prescribed without a valid practitioner-patient relationship. You agree to provide accurate and complete medical and health information.

5. Prescription Medications

You understand that if prescribed medication, it is your responsibility to: (a) follow the dosing instructions provided by your physician; (b) report any adverse effects promptly; (c) attend recommended follow-up consultations; (d) complete recommended laboratory testing; and (e) not share your medications with anyone else.

6. Laboratory Testing

You understand that laboratory testing (blood work) may be required before and during treatment. Lab draw fees are typically billed separately by the laboratory provider. You agree to complete all recommended laboratory testing to ensure the safety and efficacy of your treatment.

7. Right to Withdraw Consent

You may withdraw your consent to telehealth services at any time by contacting us at support@phlexmeds.com. Withdrawal of consent will not affect any services already provided. Upon withdrawal, your prescribing physician will provide appropriate discontinuation guidance.

8. Emergency Situations

If you experience a medical emergency, call 911 or go to the nearest emergency room immediately. Phlex Meds telehealth services are not a substitute for emergency care.
